For the safety of our staff and clients, NO CLIENTS WILL BE ALLOWED IN OUR FACILITY, and NO HOME VISITS WILL BE CONDUCTED to comply with “Social Distancing.” APPLICATION /RE-CERTIFICATION HOURS MONDAY - FRIDAY Mornings: 8:30 am – 11:30 am Afternoon 1:30 pm – 4:30 pm RE-CERTIFICATIONS:� All information may be sent electronically by email or mail. To: Justina.jones@crit-dhs.org FDP Certifier, or Victoria.enas@crit-dhs.org , FDP Manager� All re-certifications may be done over the phone (verbal) with the following guide- lines: • Social Security, G.A. with little or no changes • Physical address (no changes)• Household members (no changes) NEW CLIENT APPLICATIONS:� The FDP Certifier will follow 501 Handbook guidelines in collecting all documents for eligibility.� The FDP Certifier will make an appointment for the Household representative to have the application and all documents ready for review.� All information may be sent electronically by email or mail.� Interviews may be conducted over the phone or in the parking lot of the FDP Facility.� The FDP Certifier will review the application, and documents and make a determi- nation within the USDA/501 Handbook guidelines. (seven business days)If you or any of your family members have a cough, cold sweats, shortness of breath, or flu- like symptoms, please seek medical attention and contact the FDP Staff/Certifier.These temporary guidelines are for our staff and your safety during this time. For the safety of our staff and clients, NO CLIENTS WILL BE ALLOWED IN OUR FACILITY to comply with “Social Distancing.” Our “STORE” will be closed for the remainder of the month, March 19 - March 31.We will provide a “DRIVE UP & PICK UP SERVICE” for your monthly issuance, and you will be required to pick up ALL of your monthly issuance for March. Contact our office 24 hours in advance, and an FDP Staff will review your order with you. (Justina or Vikki). If you would like a copy of a list of food, we can mail or email you a copy.� The FDP Staff will give you a date and time (appointment) to pick up your monthly issuance.� You may pick up your order during the designated hours below at our facility.� Once you arrive at our facility, call our office, and an FDP Staff will deliver your issuance to your vehicle and assist you. MONDAY - FRIDAYMornings: 9:00 am – 11:30 amAfternoon 1:30 pm – 4:00 pm HOME DELIVERIES:� We will increase our home deliveries to accommodate more Elderly and other spe- cial needs clients. If you fall into these criteria, please contact our office, and we will address your needs.� FDP Staff will not enter your home but will leave your issuance on your front doorstep. Please contact family members to assist you in bringing in your issuance.� If no one is home, we will not leave your issuance.
