Page 9 Notice from the CRIT Enrollment Office: COLORADO RIVER INDIAN TRIBES TRIBAL MEMBER VACCINATION INCENTIVE PROGRAMMODIFICATION (February 10, 2022). Fully vaccinated Tribal members are eligible to receive cash pay- ments of $500 or $250 thanks to an incentive program just approved by the Colorado River Indian Tribes. CRIT is implementing the program using funds received by the Tribes pursuant to the American Rescue Plan Act. Every Tribal member 18 and up who receives both doses of the Pfizer or Moderna vaccines or the required single dose of the Johnson and Johnson vaccine is eligible for a one-time payment of $500. The payment is $250 for Tribal members between 5 and 17 years of age. Those who have been previously vaccinated are eligible. Tribal Council Chairwoman Amelia Flores stated, “For us to truly defeat the pan- demic, everyone who can be vaccinated should be vaccinated. Until that happens, more of our members risk becoming sick or dying. We can’t let this happen. These incentives are a wise use of dollars that provide some economic relief as well as pre- venting the spread of COVID-19." The application deadline has been extended to April 30, 2022. Submit completed Applications to the CRIT Enrollment Department, 26600 Mohave Road, Parker, AZ 85344 COVID-19 TESTING AHEAD ENROLLMENT DEPARTMENT INFORMATION: The Enrollment Department is continuously updating the current addresses of all tribal members. It is very important that each tribal member’s file records are accurate and updated. Too many addresses are being returned as insufficient, unable to forward, vacant, not deliverable as addressed. Contact the Enrollment department to update your address or verify the address on file. TRIBAL IDENTIFICATION CARDS Any tribal member requesting a new or a duplicate Tribal Identification Card will be required to call the enrollment department at (928) 669-1240, 1241, or 1304. An updated photo will be required if your photo is outdated in the computer system. The Enrollment staff is asking that you email a face photo of yourself with a plain back- ground, no hats, sunglasses, bandana, masks, or filters to the following email: enroll- ment@crit-nsn.gov The Tribal Administrative Building continues to be closed to the public until further notice. You will need to set up an appointment to pick up your Tribal ID. We thank you for your cooperation in this matter. First (1st) Tribal ID is Free of charge. Duplicate Tribal Id’s are $12.00, payable by cash, money order, cashier check, or personal check. VACCINATION APPLICATIONS The Colorado River Indian Tribes extended the vaccination applications to April 30, 2022. The vaccination application must be mailed, and a copy of your vaccination card. DO NOT send the original card. Please sign and date your vaccination application. No faxes or emails will be accepted. For those tribal members living on the reservation, a drop box is located outside in the front entrance to the Enrollment/Accounting department. You may drop your vaccina- tion application in the box provided. Checks will be mailed to the address on file with the accounting department. If you have a change of address, a W9 form will need to be completed and provided to the accounting department. Contact the accounting department to verify your address on file. TRIBAL MEMBERSHIP Currently, there are 4,533 tribal members enrolled with the Colorado River Indian Tribes.
