Page 26 21074 Mohave Road, Parker, AZ 85344 Phone: 928-662-4283 Fax: 928-662-0044 Department of Health and Social Services Mo-Chem-Ho-Na Senior Center “4 Tribes United as ONE'' The purpose of the MoChemHoNa Senior Program is to provide services to those eligible persons as mandated under Title III of the Older Americans Act. MoChemHoNa Senior Center is located 12 miles south of Parker on Mohave Road, and 2 miles north of Poston. The center is located in the most central area of the reservation and allows for accessibility to the four tribes which reside within the boundaries of the Colorado River Indian Tribes Reservation. The program is designed to enhance the lives of elders by providing nutritious meals, social activ- ities, and assistance in obtaining and receiving various supportive services. Provided Services: Congregate Meals: USDA approved meals served Mondays, Wednesdays & Fridays from 11:30pm- 1:00pm due to the Covid-19 Pandemic. Transportation: Scheduled transportation for pro- gram participants provided locally for congregate meals only due to Covid - 19 Pandemic. Caregiver Outreach: Provide/Assist with infor- mation, methods for providing support to care- givers and grandparents raising grandchildren. Homemaker Services: Is dicontinued at present time, unsure when services will resume. .Home Delivered Meals: USDA approved meals delivered to persons who meet specific eligibility criteria.
